AmeriHealth Caritas Delaware works effectively and efficiently with our providers so that together we can deliver quality, cost-effective care and better health outcomes to Delaware’s Medicaid recipients. Providers are not considered participating in the network until they have completed contracting and credentialing and have received an effective date from the plan. Existing participating providers do not need to complete credentialing unless they are due for recredentialing.

• Provider Contract Please request a contract by email or by calling your designated Account Executive.
• Provider Data Intake Form for Delaware Providers (PDF).
  For all providers, such as behavioral health and physical health providers.
• Facility Data Form for Delaware Providers (PDF).
  For all facilities, such as laboratories, long-term services and supports (LTSS) centers, and durable medical equipment (DME) centers.
